We have a limited number of
appointments each day. As a courtesy to your
fellow patients, you must have an
appointment to see your physician. If you
are experiencing symptoms, please go to the
Emergency Room. However, if your situation
is acute, call 911.
Call our office for questions or complaints related
to your cardiac condition only. Call your Primary
Care Physician or other Physicians treating you for
specific non-cardiac conditions for complaints such
as; cold, flu, arthritis, diabetes, infectious
disease, kidney problems, etc. This is especially
important with regard to medications and treatments
that another Physician has ordered and is managing.

Your First Appointment
Please arrive 30 minutes early for your first
appointment to facilitate registration. Cardiology
patients will be given a new patient questionnaire
which we request that you complete prior to your
appointment. The questionnaire requests your
personal and insurance information, as well as
medical history. For your convenience, these forms
may be downloaded and completed prior to your
appointment.

For your first appointment, please bring the
following:
- Completed questionnaire
- Current insurance cards
- Picture ID
- Medications you are taking

Subsequent Appointments
Please arrive 15 minutes early for your appointment
to facilitate registration. Also, please bring all
medication in their original prescription container
with you to your appointment.
Every effort is made to keep scheduled appointment
times. Occasionally however, a hospital emergency
might cause a delay. If this happens we will inform
you and offer you an alternative appointment if
necessary.
As part of our total commitment to your health care,
our physicians work on a rotating after-hours,
weekend and holiday schedule. In case of an
after-hours emergency, you may call the office and
the on call physician will address your needs.

Appointment Reminders

Patients will receive an automated reminder via
phone. This pre-recorded voice will remind you of
the details of your appointment in advance.

Missed Appointments
Missed appointments are one of the biggest
contributors to the inefficient operation of any
medical office. Patients who miss their appointments
consume the scarce resource of the physician’s time
while other patients must wait for appointments. In
respect for your fellow patients, please notify the
office at least 24 hours in advance if you will not
be able to keep your appointment. There will be a
$25 charge for missing an appointment without a
24-hour notice of cancellation.
